Office Policies

Late Policy If you are more than 15 minutes late for your appointment you will likely be required to reschedule and a cancellation charge may apply.

Cancellation Policy Remember that once you have made an appointment this time is reserved for you and no one else may take it. Thus a Cancellation Notice is required at least 48 hours prior to your appointment otherwise a $60.00/hour charge will be applied to your account. The same applies for Missed Appointments. We do our best to confirm appointments one week ahead and would appreciate the same respect in return if you are unable to attend. Multiple infractions will be dealt with accordingly.

Insurance Policy If you have Dental Insurance we can send most claims electronically to your Carrier. We do not take assignment of payments. Reimbursement will be sent directly to the policy holder from the Insurance Company.

Payment Policy Payment is due immediately following treatment. Payments not received immediately will be sent to a Collection Agency.

Privacy Policy PIPEDA Policy (Personal Information Protection and Electronic Documents Acts). All of our staff are aware of the sensitive nature of the information that our patients disclose. We protect our patients and the information by:

  • Only collecting necessary information
  • Not sharing any information without consent of the patient, unless directed by the law
  • Complying with existing legislation and privacy protection protocols in terms of storage, retention and destruction of patient information.
